    Mention whether you will be having your current veterinarian complete the pet health form. Include information on where your dog receives veterinary care and your commitment to good overall health and well-being.
    Please be prepared to show vaccination history records to a potential landlord if you select "Yes".

    Your willingness to have a prospective landlord meet your pet
    Assurance that your pet will make a good tenant
